Personal Release Form

By registering for this Program, I hereby acknowledge that in consideration of the Queens Borough Public Library (the “Library”) permitting me to participate in the Program I agree the following:

I have been advised by the Library to consult with my physician and describe the type of exercise instruction to be given as part of the Program, and to obtain his or her approval for me to participate in the Program, prior to participating in the Program.

I understand that participating in the Program involves the risks of personal injury, harm, and/or property damage, including but not limited to COVID-19 exposure and/or infection, and, knowing those risks, hereby assume them, and waive, release, and discharge any and all claims, including but not limited to those relating to personal injury, death or property damage, against the Library, the City of New York (the “City”), and any and all of Library’s and City’s trustees, officers, employees, affiliates, and assigns (collectively referred to as the “Released Parties”) that the Releasing Parties have or which may accrue as a result of my participation in the Program, regardless of whether such claims are caused by the negligence or carelessness of the Released Parties.

I shall indemnify and hold harmless the Released Parties from any and all liability, claims, actions, costs (including reasonable attorneys’ fees), expenses, damages, and losses (collectively referred to as “Liabilities”) that may arise from my participation in the Program, including but not limited to those related to personal injury, death, or property damage, including but not limited to COVID-19 exposure and/or infection, and regardless of whether such Liabilities are caused by the negligence or carelessness of the Released Parties.

I agree that this Release Form shall be binding on any and all of my parents, children, spouses, heirs, assigns, and estates.

I have read the above release and waiver of liability and fully understand its contents.  I voluntarily agree to the terms and conditions stated above.
